Aetrex is offering four easy options to obtaining the iStep, Lynco orthotics and footwear programs.

In order to get started with any of the programs, please complete the Customer Application by clicking on your state to download the PDF form. If your specific state is not listed below, please use the Multi-State form.

2. Flat panel Screen
3. Lap top computer
4. Printer

Option 1 - Purchase or Lease to Own
Purchase price is $5,500
Lease to own 36 monthly payments of $225

Option 2 – 12 month  Rental Program
Rent the system for 12 months for $240 per month.

Option 3 – 24 Month Rental Program
Rent the system for 24 months and receive 4 Free Lynco Orthotics each month for 24 months, 96 total pair at no charge.

Option 4: Get Started with Shoes or Lynco’s
Order any Aetrex shoe or Lynco as needed by opening an account with Aetrex.
Download  the Customer Application and Credit app grid.

Please also complete the Evolution Email Setup Form when submitting any agreement and customer credit application
